    What makes Louisiana Medicaid different

    Louisiana's Medicaid program is jointly funded by the state and federal government but administered by the state under federally set parameters. Most persons receive health services via a Managed Care Program called Healthy Louisiana (formerly Bayou Health). In addition to nursing home coverage, LA Medicaid pays for care services and supports to help frail seniors continue to live at home, in adult foster care, or in assisted living.

    Entitlement

    Institutional / Nursing Home Medicaid

    An entitlement; anyone who is eligible will receive assistance. Benefits are provided in Medicaid-certified nursing homes.

    Limited Slots

    Medicaid Waivers / Home and Community Based Services (HCBS)

    Not an entitlement; there are a limited number of participant enrollment slots and wait lists may exist. Intended to delay and prevent nursing home admissions, services are provided at home, adult day care, adult foster care, or in assisted living.

    An entitlement; anyone who meets the requirements will receive assistance. While Regular Medicaid is not Long-Term Care Medicaid, some long-term care benefits, such as personal care assistance or adult day care, may be available.

    *All of a beneficiary's monthly income, with the exception of a Personal Needs Allowance of $45 / month, Medicare health premiums, and possibly a Needs Allowance for a non-applicant spouse, must be paid to the nursing home. This is called a Patient Liability.

    Based on one's living setting, a program beneficiary may not be able to keep monthly income up to this level.

    Key Rules at a Glance

    • Nearly all income received is counted, including cash from family/friends, employment wages, alimony, pension payments, SSDI, SSI, IRA withdrawals, and stock dividends.
    • Nationally, Holocaust restitution payments are not counted as income.
    • In LA, the VA Aid & Attendance, which is above and beyond the Basic VA Pension, does not count.
    • When only one spouse applies for Nursing Home Medicaid or a Medicaid Waiver, only the applicant's income is counted.
    • The MMNA in LA is $4,066.50 / month (2026) — set at the federal maximum. If a non-applicant's income is under this amount, income can be transferred from the applicant spouse.
    • A non-applicant spouse with $4,066.50 or more in monthly income is not entitled to a MMNA / Spousal Income Allowance.
    • For Regular Medicaid, both spouses' income is counted. There is no MMNA for a non-applicant spouse.
    • In Louisiana, persons eligible for SSI are automatically approved for Medicaid, including long-term care via Regular Medicaid given functional need requirements are met.
    • Countable assets include cash, stocks, bonds, investments, cryptocurrency, bank accounts (money market, savings, checking), and real estate in which one does not reside.
    • In Louisiana, IRAs and 401K's are counted as assets.
    • Non-countable (exempt) assets include personal belongings, household furnishings, an automobile, irrevocable burial trusts, life insurance policies (combined face value limit of $10,000), and generally one's primary home.
    • All assets of a married couple are considered jointly owned regardless of the Medicaid program.
    • The CSRA allows the non-applicant spouse to keep up to $162,660 of the couple's assets (2026).
    • There is no CSRA for Regular Medicaid.
    • The home is automatically exempt if the applicant's spouse, child under 21, or permanently blind or disabled child (of any age) lives in it.
    • If this is not the case, there is a home equity interest limit of $730,000 (in 2026).
    • Home equity is the value of the home minus any outstanding debt against it. Equity interest is the amount of equity owned by the applicant.
    • If neither the applicant nor qualifying family live in the home, the applicant must have Intent to Return.
    • There is no home equity interest limit for Regular Medicaid.
    • The home is NOT exempt from Medicaid's Estate Recovery Program. After a beneficiary's death, Louisiana's Medicaid agency attempts reimbursement of care costs through the deceased's estate.
    • Louisiana has a 60-month Look-Back Period for Nursing Home Medicaid and Medicaid Waiver applications that immediately precedes the application date.
    • During the look-back, Medicaid checks to ensure no assets were sold or gifted under fair market value, including transfers by one's spouse.
    • Violations result in a Penalty Period of Medicaid ineligibility.
    • The Look-Back Rule does NOT apply to Regular Medicaid.
    • The U.S. Federal Gift Tax Rule ($19,000 per recipient in 2026) does not extend to Medicaid eligibility — gifting under this rule still violates the Look-Back Period.
    • MMMNA: $4,066.50 · CSRA: $162,660 · CSRA Min: $32,532
    • When only one spouse applies for Nursing Home Medicaid or a Medicaid Waiver, only the applicant's income is counted.
    • Louisiana sets the MMNA at the federal maximum of $4,066.50 / month (2026). If the non-applicant's income is under this amount, income can be transferred from the applicant spouse.
    • The CSRA allows the non-applicant spouse to keep up to $162,660 of the couple's assets (2026).
    • For Regular Medicaid, both spouses' income is counted and there is no MMNA or CSRA.
    • Medically Needy Pathway — Louisiana has a Medically Needy Program (MNP) / 'Spend-Down' program. The medically needy income eligibility standard (MNIES) varies by area: Urban (East Baton Rouge, Jefferson, Orleans, St. Bernard Parishes) is $100 / month individual, $192 / month couple. Rural areas: $92 / month individual, $167 / month couple. The spend-down is calculated for a 3-month period. Waiver beneficiaries can retain a Basic Needs Allowance of $2,982 / month. The medically needy asset limit is $2,000 individual, $3,000 couple.
    • Asset Spend-Down — Spend excess assets on non-countable items such as home modifications (wheelchair ramps, stair lifts, widened doorways, first-floor bedrooms/bathrooms), prepaying funeral/burial expenses, and paying off debt. Keep documentation as proof the Look-Back Rule was not violated.
    • Medicaid Planning — Work with a Medicaid Planning Professional to employ strategies to become eligible and protect your home from Medicaid's Estate Recovery Program.

    Louisiana does not use Qualified Income Trusts (Miller Trusts). The Medically Needy Program is available for persons over the income limit, with income limits varying between urban and rural parishes.

    Adult Day Health Care (ADHC) Waiver

    Also called the Adult Day Care Waiver, adult day care is provided for Louisiana residents who are elderly or disabled.

    LT-PCS (Personal Care Services) Program

    An entitlement program through Louisiana's Medicaid State Plan. Assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), such as bathing, grooming, mobility, eating, laundry, and shopping for essentials, is provided in one's home.

    Community Choices Waiver (CCW)

    Intended for adults with physical disabilities and seniors who would require nursing home placement without the program's services. Benefits include respite care, adult day care, temporary skilled nursing, and assistive devices, available at home, in assisted living, or adult foster care.

    Wait List

    Program of All-Inclusive Care for the Elderly (PACE)

    Through PACE, the benefits of Medicare and Medicaid, including long-term care, are available via a single program. Additional benefits, such as dental and eye care, may be available.

    Money Follows the Person (My Place Louisiana)

    A federal program that helps institutionalized Medicaid-eligible persons transition back home or into the community. Known in Louisiana as My Place Louisiana.
