We are LTCareNav, LLC ("LTCareNav," "we," or "us"). Our Service includes our website at www.LTCareNav.com, and our related websites and other services provided by, or operated on behalf of, us and where this Privacy Policy is linked (collectively, the "Service").
We provide the Service to our users and their families, as well as to participating communities, home care agencies, and other Commercial Users (defined below and further discussed in our Terms of Services) that have been vetted and approved by LTCareNav. Certain aspects of the Services are made available for use by participating communities and home care agencies ("Commercial Users"). This Privacy Policy applies to such Commercial Users of our Service.
By accessing and using the Service, which includes any web or mobile services that we may make available to you ("you" and "your"), you are accepting the practices described in this Privacy Policy and are also agreeing to comply with our Terms of Service. Capitalized terms used in this Privacy Policy but not defined here have the meanings ascribed to them in the Terms of Service.
Before submitting any information which may identify you ("personally identifiable information") to our Services, please read this Privacy Policy carefully. This Privacy Policy is designed to assist you in understanding how we collect, use, and disclose both personally identifiable information and all other information (collectively "Information") that we obtain through your use of our Service.
Our Service provides information to you about our products and services. When you use our Service, Information will be automatically collected using cookies and similar technologies. We may ask your consent when required.
What You Should Know Before Using the LTCareNav Service
Before using and sharing Information through our Service, you should understand the following.

Sources and Types of Personally Identifiable Information We Collect from You
We receive and store any Information you enter in our Service or give us in any other way. We also collect certain Information automatically when you use our Service. If you choose not to provide this Information to us, you may not be able to obtain the benefits of the services as a user of our Service.
Information we collect from users of our Service includes Information that will be collected automatically using cookies and similar technologies. Cookies collect different aspects of information, including personally identifiable information. This kind of Information may include IP addresses, session information, product usage data, tracking data and other information about your use of our Service.
When you are a registered user of our Service, we need to collect more Information to allow you to use the features and functions of our Service. We will need your email address, age, and will request your name, address, telephone, general financial information, job title, information regarding your business, and optionally an image of you. LTCareNav uses the Information we collect about you for registration and to manage your account, to communicate with you, and to connect you with potential clients and residents.
We may use your Information as defined in the Terms of Service and to provide and/or improve the Service. We may use your Information in an anonymized manner to develop additional products and services or to measure and improve our products and services and to develop or improve third party products and services.
We may use automatically collected information, including through cookies, to: (i) conduct analyses about our Services; (ii) associate the different devices you use; (iii) remember your browser or device; (iv) remember your preferences and interactions; and (v) log activity on our Services.
We use your Information when you authorize us to use it and when it is necessary to enforce this Privacy Policy and our Terms of Service. These purposes may include the following:
- for registration and to manage your account, including to facilitate your access to and use of our Service;
- to communicate with you and respond to your questions and comments;
- to enable us to deliver our products and services to you;

Use of Artificial Intelligence
We use artificial intelligence, machine learning models, and related automated technologies ("AI") to support certain features of our Service, including generating content, summarizing information, powering conversational features, and assisting with search or matching functionality. AI may be provided by LTCareNav directly or by third-party vendors whose technology we incorporate into the Service.
As a Commercial User, the Information you provide to us, including information about your community, agency, services, pricing, availability, and staff, may be processed by AI to generate shortlists, sort orders, matches, or connection options shown to users and their families who are searching for care. AI-generated outputs are provided for convenience to the consumers using our Service and are not endorsements, guarantees, or recommendations by LTCareNav.
When your Information is processed by an AI-powered feature, it may be processed by LTCareNav and by the third-party AI vendor(s) supporting that feature. We require our third-party AI vendors to protect the confidentiality and security of the Information we share with them, consistent with the agreements described in "How We Disclose Your Information" below.
We do not use the Information you provide to AI-powered features to train or fine-tune any third-party AI vendor's models for the benefit of other parties, except as may be otherwise disclosed to you at the time of collection or in a separate written agreement between LTCareNav and your organization.
If you are an employee, contractor, or agent of a Commercial User, your individual Information (such as your name, job title, or contact information) may also be processed by AI-powered features in connection with your employer's account, in the same manner described above. This Use of Artificial Intelligence section should be read together with "Your European Privacy Rights" below regarding the data processor and data controller roles of LTCareNav and your employer.
Retention of Information processed by AI-powered features is governed by the "Retention Periods and Deletion" section below.
How We Disclose Your Information
Some of your Information may be disclosed with potential clients and residents that have been matched with you. Additionally, if you are an employee, contractor or agent of a LTCareNav business customer, we may disclose your Information with your applicable employer, such as, but not limited to, when managing all active accounts under your employer's subscription with us. LTCareNav is not responsible or liable for any uses of your Information or your content by your employer.
We may disclose aggregated data and high-level information in a way that does not identify individuals.
We do disclose your Information for processing and storage with third parties that have agreements in place with us to protect the confidentiality and security of information that we transfer to them. This includes third-party AI vendors that process Information on our behalf to support AI-powered features, as described in "Use of Artificial Intelligence" above.
We may disclose your personally identifiable information for lawful purposes, including in the following situations:
- We believe that disclosure is reasonably necessary to comply with any applicable law, regulation, subpoena, or court order;
- To respond to duly authorized information requests from law enforcement or other governmental authorities;
- To enforce our agreements or policies;
- To investigate and prevent security threats, fraud, or other malicious activity; or
- To respond to an emergency that we believe in good faith requires us to disclose such information to assist in preventing the death or serious bodily injury of any person or an LTCareNav employee.
There are circumstances where we may decide to buy, sell, or reorganize its business. Under these circumstances, it may be necessary to disclose or receive personally identifiable information with prospective or actual purchasers, acquisition targets, partners or affiliates. In such circumstances, we will ensure your information is used in accordance with this Privacy Policy.
Your European Privacy Rights
Under the (i) General Data Protection Regulation (Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016, or "GDPR"), (ii) Data Protection Act 2018, (iii) the GDPR as it forms part of the law of England and Wales, Scotland and Northern Ireland (i.e., "UK GDPR") as provided in the Data Protection Act 2018, and (iv) any other applicable data protection legislation of any country or other jurisdiction (collectively "International Data Protection Laws"), individuals have specific rights with respect to their personally identifiable information, or "personal data" as defined under the International Data Protection Laws. For the purposes of this Privacy Policy, LTCareNav operates as a data processor. Any personal data we collect from you is processed in the United States and under the terms of this Privacy Policy.
Any personal data we collect from you is processed in the legitimate interest of our business and providing our services to you as the lawful means of such processing. You may always withdraw your consent to our use of your personal data. We will only retain your personal data for the time necessary to provide you the information and services to which you have consented, to comply with the law and in accordance with your rights below.
Where AI-powered features described in "Use of Artificial Intelligence" above process your personal data, LTCareNav and its third-party AI vendors act as processors or sub-processors on behalf of the Data Controller identified below, and process such data only as necessary to provide the Service.

The Data Controller is your employer that has a subscription with us. LTCareNav is a "Service Provider" or "Processor" as defined by the International Data Protection Laws. To the extent International Data Protection Laws allow you to make certain requests, including requests to access, correct, erase personal data, or withdraw consent, you must make these requests directly to your employer. If you submit a request to LTCareNav regarding your rights under applicable International Data Protection Laws, we may redirect you to your employer.
Your Preferences
From time to time, we may send you email newsletters and marketing emails. You may opt out of them at any time by selecting the "unsubscribe" link at the bottom of each email. Please note that by opting out or unsubscribing you may affect other services you have requested we provide to you, in which email communication is a requirement of the service provided. Even if you opt-out of receiving marketing material, we may still need to contact you with important information about your account or responding to your requests or questions.
Our Services may use Facebook Connect, Google Analytics, Google Audiences, Meta, and others for audience measurement and analytics purposes. These analytics services may use cookies or similar technologies to collect information to help us analyze users and how they use our Services. The Information collected by these technologies is used to assess how often you visit our Services, what pages you view when you visit our Services, and what other websites you visited before coming to our Services. Information generated by these services may be transmitted to and stored by these providers and they may use this Information for purposes such as evaluating your use of our Services, compiling statistics reports on the Services' activity, and providing other services relating to our Services activity and other internet usage. Do Not Track Technologies
Some browsers may have settings to request that websites "Do Not Track" your activity or usage. At this time, we do not use services that will accommodate these technologies and your requests may not be able to be honored on our Service. Retention Periods and Deletion
LTCareNav retains your Information to the extent necessary to reasonably provide the Services and the Service requirements, to meet our compliance and legal obligations, to enhance security and fraud prevention, and for audit purposes. We will keep your Information until you no longer participate as a user of the Service and in accordance with our Terms of Service.

We want you to feel confident about using our Service, and we are committed to protecting the personally identifiable information we collect.
