What You Should Know Before Using the LTCareNav Service

Before using and sharing Information through our Service, you should understand the following.

Sources and Types of Personally Identifiable Information We Collect from You

We receive and store any Information you enter in our Service or give us in any other way. We also collect certain Information automatically when you use our Service. If you choose not to provide this Information to us, you may not be able to obtain the benefits of the services as a user of our Service.

Information we collect from users of our Service includes Information that will be collected automatically using cookies and similar technologies. Cookies collect different aspects of information, including personally identifiable information. This kind of Information may include IP addresses, session information, product usage data, tracking data and other information about your use of our Service.

How We Use Your Information

LTCareNav uses the Information we collect about you for registration and to manage your account, to communicate with you, and to connect you with resources and appropriate providers and professionals. We will also use your Information to allow those providing the Service to you to have the information necessary to answer your questions and to respond to your requests.

We may use your Information as defined in the Terms of Service and to provide and/or improve the Service. We may use your Information in an anonymized manner to develop additional products and services or to measure and improve our products and services and to develop or improve third party products and services.

Use of Artificial Intelligence

We use artificial intelligence, machine learning models, and related automated technologies ("AI") to support certain features of our Service, including generating content, summarizing information, powering conversational features, and assisting with search or matching functionality. AI may be provided by LTCareNav directly or by third-party vendors whose technology we incorporate into the Service.

When you submit Information to an AI-powered feature, that Information may be processed by LTCareNav and by the third-party AI vendor(s) supporting that feature, in order to generate the relevant content, response, or match. We require our third-party AI vendors to protect the confidentiality and security of the Information we share with them, consistent with the agreements described in "How We Disclose Your Information" below.

We do not use the Information you submit to AI-powered features to train or fine-tune any third-party AI vendor's models for the benefit of other parties, except as may be otherwise disclosed to you at the time of collection.

AI-assisted features, including AI-assisted matching, may be used to identify and suggest Commercial Users or other providers that may be relevant to your stated needs and preferences. As described in our Terms of Service, this may involve sharing your contact information and a general overview of your care needs with one or more Commercial Users so that they may follow up with you. AI-generated outputs are provided for convenience and are not a substitute for your own judgment or independent evaluation.

Retention of Information processed by AI-powered features is governed by the "Retention Periods and Deletion" section below.

How We Disclose Your Information

Your Information is only disclosed with family, professional(s), provider(s) and to the extent that you configured our Service to disclose. That person can only see the Information you have submitted to our Service to help the member assist you with your problem or need, both as requested by you and determined by our system.

We may disclose aggregated data and high-level information in a way that does not identify individuals.

We do disclose your Information for processing and storage with third parties that have agreements in place with us to protect the confidentiality and security of information that we transfer to them. This includes third-party AI vendors that process Information on our behalf to support AI-powered features, as described in "Use of Artificial Intelligence" above.

We may disclose your personally identifiable information for lawful purposes, including in the following situations:

• We believe that disclosure is reasonably necessary to comply with any applicable law, regulation, subpoena, or court order;
• To respond to duly authorized information requests from law enforcement or other governmental authorities;
• To enforce our agreements or policies;
• To investigate and prevent security threats, fraud, or other malicious activity; or
• To respond to an emergency that we believe in good faith requires us to disclose such information to assist in preventing the death or serious bodily injury of any person or an LTCareNav employee.

There are circumstances where we may decide to buy, sell, or reorganize its business. Under these circumstances, it may be necessary to disclose or receive personally identifiable information with prospective or actual purchasers, acquisition targets, partners or affiliates. In such circumstances, we will ensure your information is used in accordance with this Privacy Policy.

Your European Privacy Rights

While we do not provide services to or market the Service to those who reside in the Europe Economic Area (the "European Union" or "EU"), we will provide accommodations for the exercise of your rights provided under data protection laws of the EU, including the General Data Protection Regulation (the "GDPR").

If you are entitled to privacy rights under the GDPR, you are entitled to:

Transparency and the right to information. Through this Privacy Policy we explain how we use and disclose your information. However, if you have questions or concerns you can contact us any time.

AI-assisted features described in "Use of Artificial Intelligence" above may suggest matches or content based on your Information, but final decisions about acceptance, services, and pricing remain with you and the relevant Commercial User, as described in our Terms of Service. We use cookies and similar technologies in our Service to personalize your experience of our Service.

Right to lodge a complaint with a supervisory authority. If you consider that the processing of your personal data infringes your privacy rights according to the European Data Protection law, you have the right to lodge a complaint with a supervisory authority, in the member state of your habitual residence, place of work, or place of the alleged infringement.

Your Preferences

From time to time, we may send you email newsletters and marketing emails. You may opt out of them at any time by selecting the "unsubscribe" link at the bottom of each email. Please note that by opting out or unsubscribing you may affect other services you have requested we provide to you, in which email communication is a requirement of the service provided. Even if you opt-out of receiving marketing material, we may still need to contact you with important information about your account or responding to your requests or questions.

Our Services may use Facebook Connect, Google Analytics, Google Audiences, Meta, and others for audience measurement and analytics purposes. These analytics services may use cookies or similar technologies to collect information to help us analyze users and how they use our Services. The Information collected by these technologies is used to assess how often you visit our Services, what pages you view when you visit our Services, and what other websites you visited before coming to our Services. Information generated by these services may be transmitted to and stored by these providers and they may use this Information for purposes such as evaluating your use of our Services, compiling statistics reports on the Services' activity, and providing other services relating to our Services activity and other internet usage. At this time, we do not use services that will accommodate these technologies and your requests may not be able to be honored on our Service. Retention Periods and Deletion

LTCareNav retains your Information to the extent necessary to reasonably provide the Services and the Service requirements, to meet our compliance and legal obligations, to enhance security and fraud prevention, and for audit purposes. We will keep your Information until you no longer participate as a user of the Service and in accordance with our Terms of Service.
