Refining Your LTCareNav Profile: Go Deeper for More Personalized Care Planning

Refining Your LTCareNav Profile: Go Deeper for More Personalized Care Planning

There's no single "right way" to start planning with LTCareNav. You may have begun with the Short LTC Plan, or with any of our other planning tools, assessments, or resources—whatever fit you best at the time. If you started with a tool, that's what you'll see when you first enter your dashboard. If you came in through a resource, your dashboard may start in an empty state—and that's perfectly fine. You can begin filling in your plan right there, or use the planning tools and watch everything populate into your dashboard automatically.

Here's the part most people don't expect: there isn't one short quiz and one long assessment. There's a whole set of assessments and tools, and each one you complete builds your dashboard and your plan. Do the ones that matter to you. Ignore the ones you don't need. Come back when you need them. Every detail you add makes your projections more accurate—and it's all free, at your own pace.

    Personal and Demographics

Whichever tools you've used so far have already given your dashboard a starting point. This section lets you add more context—your marital status, household information, and family or support system—which plays a big role in shaping your care projections.

What you'll need if you choose to fill in this section:

    • Your date of birth
    • Current address
    • Marital status
    • Information about household members or caregivers

    Health and Functional Status

Any health-related assessments you've completed give us a snapshot of your physical and cognitive health. This section lets you go deeper—adding chronic conditions, mobility details, medications, and how much support you need with everyday tasks—so your care projections reflect your real situation.

What you'll need if you choose to fill in this section:

    • A list of current medical diagnoses (or access to your medical records)
    • Notes on any recent hospitalizations or care needs
    • Information about medications, mobility aids, or cognitive assessments

    Insurance Coverage

This section lets you tell us about any insurance you currently have—like Medicare, Medicaid, private health insurance, or long-term care coverage. Adding this detail helps us assess how your coverage may help offset care costs, giving you a clearer financial picture.

What you'll need if you choose to fill in this section:

    • Medicare and/or Medicaid ID numbers (if applicable)
    • Insurance cards or plan documents
    • Long-term care insurance policy details, if you have one

    Financial Situation

If you've used any of our financial tools, your dashboard may already have broad ranges for income, assets, and home value. This section lets you replace those estimates with your actual numbers—giving you a much more realistic picture of what care you can afford and how long your resources might last.

What you'll need if you choose to fill in this section:

    • Monthly income (from Social Security, pensions, work, etc.)
    • Estimated value of savings and assets (like a home or retirement accounts)
    • Monthly expenses (housing, insurance premiums, medical bills, etc.)

    Care Preferences

This is where you tell us what you want for yourself. Whether it's staying at home, living near family, or planning for more support down the road, this section helps tailor your recommendations to what matters most to you.

    What to think about:

    • Do you want to stay in your home as long as possible?
    • Would you consider assisted living or in-home care?
    • What type of setting makes you feel safe and supported?

Every Tool You Use Makes Your Plan Better

You don't have to complete everything to see value. Each assessment you finish and each section you fill in makes your care scenarios, financial projections, and Medicaid timelines more accurate. Start with whatever matters most to you right now, and come back to the rest whenever you're ready—your dashboard keeps everything in one place, waiting for you.

    Your Data Is Always Secure

We know this is a lot of information to share—and we don't take that lightly. Everything you enter is encrypted, stored securely, and never shared. Privacy and protection are at the core of everything we do!

Go at Your Own Pace—And Come Back Anytime
