LTCareNav Respite Care Giveaway — Official Terms

1. Sponsor

This Giveaway ("Giveaway") is sponsored by LTCareNav, LLC, an Ohio limited liability company, PO Box 140, 30799 Pinetree Road, Cleveland, Ohio 44124 ("Sponsor," "we," "us").

2. Giveaway Period

The Giveaway begins October 1, 2026 at 12:00 AM ET and continues until twenty (20) winners have been selected or 11:59 PM ET on June 1, 2027, whichever comes first ("Giveaway Period"). A drawing will be held each time the total number of eligible entries received reaches the next multiple of two hundred fifty (250) (that is, at 250, 500, 750, and so on, up to 5,000 eligible entries), with one (1) winner selected at each drawing, for a maximum of twenty (20) winners. Each period between drawings is a "Drawing Period." Sponsor will post the current number of eligible entries and the date of each drawing on the Giveaway page. Sponsor reserves the right to end, pause, or modify the Giveaway at any time, as described in Section 11.

3. Eligibility

To be eligible to enter and win, an entrant must, at the time of entry and at the time of winner selection:

• Be at least 18 years of age or the age of majority in their state of residence, whichever is older;
• Be a legal resident of the United States, including the District of Columbia;
• Hold an active LTCareNav family account in good standing, or have entered by the Alternate Method of Entry in Section 4;
• Be a family caregiver for, or represent a household that includes, a care recipient who lives at home (not in a facility) in the United States, including the District of Columbia, and who has a family caregiver who would benefit from respite support;
• Not be an employee, officer, director, or contractor of LTCareNav, its affiliates, or its respite/companion care partner network, or a member of such person's immediate family or household;
• Not have previously won this Giveaway. Each family/household may win one time only, for the life of the Giveaway. A prior winner remains a valued LTCareNav member but is not eligible for future drawings.

Sponsor reserves the right to verify eligibility at any stage and to disqualify any entrant who does not meet these requirements or who provides false or inaccurate information. Each potential winner must certify the requirements above in the Prize Acceptance described in Section 5.

Eligibility is limited to entrants whose care situation falls within the scope of companion-level care. "Companion-level care" means non-medical support that does not involve hands-on physical assistance, such as companionship, supervision, conversation and activities, meal preparation, light housekeeping, errands, and verbal reminders. It does not include bathing, toileting, dressing, transferring, lifting, or other hands-on personal care; administering or handling medication; wound care; or any skilled, medical, or clinical service. This Giveaway does not provide, and is not intended to substitute for, skilled nursing, medical, or clinical care. If an entrant's needs fall outside this scope, Sponsor or the Provider may deem the entrant ineligible for the prize; Sponsor may, at its discretion, offer alternative resources or support to that family outside the scope of this Giveaway.

This Giveaway is intended for LTCareNav members. Online entry requires signing up for a free LTCareNav family account. There is no cost to create an account and no cost to enter. Entrants may also enter by the Alternate Method of Entry in Section 4.

4. How to Enter

No purchase necessary to enter. Entry requires becoming an LTCareNav family member (free). You may also enter without creating an account by the Alternate Method of Entry below.

1. Sign up for a free LTCareNav family account, or log in to an existing account, and complete the eligibility quiz available at ltcarenav.com/winfreerespite;
2. Affirmatively agree to these Official Terms, the LTCareNav Terms of Use (ltcarenav.com/terms-of-use), the LTCareNav Privacy Policy (ltcarenav.com/privacy-policy), and the LTCareNav Consumer Health Data Privacy notice (ltcarenav.com/consumer-health-data-privacy).

Upon completing both steps, the entrant is automatically entered into the drawing for the current and each subsequent drawing for as long as their LTCareNav account remains active and they continue to meet eligibility requirements (including the one-win-per-household limit in Section 3), without the need to re-enter.

Alternate Method of Entry (AMOE). To enter without completing the online quiz or creating an account, hand-print your name, mailing address, email address, phone number, date of birth, and the state where the care recipient lives on a 3" x 5" card and mail it in a sealed, stamped envelope to: LTCareNav Respite Giveaway, PO Box 140, 30799 Pinetree Road, Cleveland, Ohio 44124. A mail-in entry is included in the drawing for the Drawing Period in which it is received and each later drawing, with the same chance of winning as an online entry. Limit one mail-in entry per person and household; mechanically reproduced entries are void.

Limit: one entry per person, per household. Multiple or duplicate entries by the same individual will be void.

5. Winner Selection and Notification

Each drawing will be held within seven (7) days after the total number of eligible entries reaches the next multiple of two hundred fifty (250). At each drawing, one (1) winner will be selected by random drawing from all eligible entries received up to that point, excluding any entrant who has already won. Eligible entries that do not win remain in all later drawings. If the Giveaway Period ends before twenty (20) winners have been selected, Sponsor will hold one final drawing from all eligible entries received, and no further Prizes will be awarded. Winners will be notified by email and/or phone using the contact information provided at entry, within three (3) business days of selection. Winner must respond and confirm eligibility within 72 hours of notification, or an alternate winner may be selected. Before any care is scheduled, the winner and the care recipient (or the care recipient's legally authorized representative) must sign and return a Prize Acceptance, Release, and Assumption of Risk form within seven (7) days after receiving it, or the Prize may be forfeited and an alternate winner selected.

Odds of winning depend on the total number of eligible entries received at the time of each drawing. For the names of winners, send a self-addressed, stamped envelope marked "Respite Giveaway Winners List" to the address in Section 14.

6. Prize

Each winner will receive a prepaid credit for four (4) hours of companion-level respite care (a "Care Credit"), purchased by Sponsor and redeemable with an independent third-party home care provider participating in Sponsor's partner network, including providers made available through Respitely (respitelycares.com) (each, a "Provider") ("Prize"). The value of the Prize will reflect the prevailing local market rate for four hours of companion care in the winning family's geographic area, and will therefore vary by location. The approximate retail value ("ARV") of each Prize is $120–$225, depending on location. Sponsor will introduce the winner to one or more available Providers, and where more than one is available, the winner will choose the Provider. The winner schedules care directly with the Provider, and care is provided under the Provider's own client service agreement, intake assessment, and policies, which the winner (and care recipient, as applicable) must accept. The Provider may, in its sole judgment, decline to provide or may discontinue services if the care recipient's needs exceed companion-level care or the Provider believes the setting is unsafe. The Care Credit must be scheduled and used within sixty (60) days after the winner is confirmed, may be used in up to two (2) visits, and unused hours are forfeited without substitution.

Sponsor will make reasonable efforts to fulfill the Prize as in-kind respite care through its partner network before offering any substitute. If, after those efforts, Sponsor is unable to identify or coordinate an available network provider in the winning family's area, or if the Provider declines to serve the care recipient, Sponsor will instead issue the winner a $160 gift card to be used toward respite care of their choosing, in lieu of coordinated care. This substitution is at Sponsor's sole discretion and reflects Sponsor's good-faith fallback, not a guaranteed equivalent value in every market. Sponsor has no responsibility for how the gift card is used or for any care purchased with it. The ARV of each Prize, including any gift card substitute, will not exceed two hundred twenty-five dollars ($225), and the total ARV of all Prizes in this Giveaway will not exceed four thousand five hundred dollars ($4,500).

The Prize does not include, and Sponsor is not responsible for, hands-on personal care, medical, clinical, or skilled nursing care of any kind, or emergency response. In an emergency, call 911. Prize is non-transferable and no cash or other substitution will be made except as expressly described in this Section 6. Sponsor reserves the right to substitute a comparable prize if the stated Prize becomes unavailable.

Winner is solely responsible for any applicable federal, state, or local taxes associated with the Prize.

7. Third-Party Care Providers; Insurance

Each Provider is an independent business and is not an agent, employee, partner, or joint venturer of Sponsor. Each Provider has represented to Sponsor that it is licensed or registered where required, screens and background-checks its caregivers, and maintains its own general liability, professional liability, and other applicable insurance, and each Provider is solely responsible for doing so. Sponsor does not independently verify, and does not guarantee, any caregiver's qualifications, background, or insurance. Sponsor's role is limited to purchasing the Care Credit and introducing the winner to available Providers. Sponsor does not itself provide care, does not employ, supervise, direct, or control the caregivers who deliver the Prize or the care they provide (consistent with Sections 5 and 10 of the LTCareNav Terms of Use), and assumes no responsibility for the acts, omissions, quality of care, or conduct of any third-party provider. Entrants and winners acknowledge and agree that any claim arising from care received must be directed to the provider and its insurer, not to Sponsor, except to the extent caused by Sponsor's own gross negligence or willful misconduct.

The winner and care recipient are responsible for giving the Provider accurate information about the care recipient's needs, conditions, and home, and for keeping a family contact reachable during each visit. The winner and care recipient voluntarily assume the risks of receiving in-home care from a third-party Provider.

8. Publicity Release

As a condition of accepting the Prize, each winner must sign a publicity release, except where prohibited by law, consenting to Sponsor's use of the winner's first name, city/state, and statements regarding their experience for promotional purposes related to this Giveaway, without additional compensation, unless otherwise required by law. Winners may also be asked, but are never required as a condition of receiving or keeping the Prize, to write a review of their LTCareNav experience. Sponsor will not use the care recipient's name, image, or health information without the separate written consent of the care recipient or their legally authorized representative.

9. Privacy

Information collected in connection with this Giveaway will be handled in accordance with the LTCareNav Privacy Policy (ltcarenav.com/privacy-policy) and Consumer Health Data Privacy notice (ltcarenav.com/consumer-health-data-privacy). By entering, entrants consent to the collection and use of their information as described there and in these Official Terms. Sponsor will share the winner's and care recipient's names, contact information, address, and relevant care-needs information with the selected Provider (and Respitely, if applicable) solely to fulfill the Prize. An entrant who deletes their LTCareNav account, or withdraws consent to the processing of their consumer health data, is no longer eligible for future drawings.

10. Release and Limitation of Liability

By entering, entrants agree to release and hold harmless Sponsor and its parents, subsidiaries, and affiliates, and each of their respective officers, directors, members, managers, employees, and agents (the "Released Parties"), from any and all liability, injury, loss, or damage arising from participation in this Giveaway or acceptance, receipt, or use of the Prize, including any liability related to the care provided by a third-party companion care partner. See also Section 7. The Prize Acceptance will include the same release from the care recipient. The Disclaimer and Limitations on Our Liability and Indemnification provisions of the LTCareNav Terms of Use (Sections 17 and 18) also apply to this Giveaway. TO THE FULLEST EXTENT PERMITTED BY LAW, THE RELEASED PARTIES WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM THE GIVEAWAY OR THE PRIZE. Nothing in these Official Terms limits liability that cannot be limited under applicable law, including for gross negligence or willful misconduct.

11. Right to Cancel, Modify, or Disqualify

Sponsor reserves the right, at its sole discretion and at any time, to cancel, suspend, pause, modify, amend, or end the Giveaway, or these Official Terms, for any reason, including if the Giveaway is not capable of running as planned due to fraud, technical failure, or any other cause. If Sponsor elects to end the Giveaway, Sponsor will provide reasonable notice on the Giveaway page. Sponsor reserves the right to disqualify any entrant found to be tampering with the entry process or acting in violation of these Official Terms. Sponsor will not modify these Official Terms in a way that materially reduces an entrant's chance of winning in a Drawing Period already under way, except as required by law or to address fraud or technical failure, and will post any change with a revised "Last updated" date.

12. Governing Law and Disputes

This Giveaway and these Official Terms are governed by the laws of the State of Ohio, without regard to conflict-of-law principles. Any dispute arising out of or relating to this Giveaway or the Prize will be resolved under the Dispute Resolution & Arbitration provisions (Section 19) of the LTCareNav Terms of Use, including binding individual arbitration administered by the American Arbitration Association, the class action and jury trial waivers, the informal dispute resolution requirement, and the opt-out procedure described there. To the extent any claim may proceed in court, exclusive venue is in the state or federal courts located in Cuyahoga County, Ohio.

13. Void Where Prohibited

This Giveaway is void where prohibited or restricted by law.
