Moving a Loved One Across States Takes More Than Finding a Home
Moving an aging or vulnerable loved one across state lines isn't as simple as finding a place for them to live.
In this real-life example, a sister reached out because her brother was living out of state, had medical needs, and didn't have family nearby to oversee what was happening.
The first step wasn't finding a community. It was figuring out what needed to happen first.
That meant understanding his medical needs, getting the right documentation, helping his sister establish the ability to act on his behalf, cleaning out his apartment, arranging the right transportation, coordinating a rehabilitation stay, and making sure he had a safe place to live when he arrived.
Even transportation required planning. A 12-hour car ride wasn't realistic for him, so the family explored medical transportation and ultimately arranged private air transportation with caregivers.
And when he arrived, there were already doctors, rehabilitation, therapy, and a care setting in place.
That's what comprehensive care coordination can look like.
When a family is overwhelmed, the answer isn't always "find a place." Sometimes you have to work backward and figure out every step that needs to happen to get someone safely from where they are to where they need to be.
