LTCareNav and Care Bloom 2026 Giveaway — Official Terms

1. Sponsor

This giveaway ("Giveaway") is sponsored by LTCareNav, LLC, an Ohio limited liability company, PO Box 140, 30799 Pinetree Road, Cleveland, Ohio 44124 ("Sponsor," "we," "us"), in collaboration with Care Bloom, Inc., a Delaware corporation, 3401 Enterprise Parkway, Suite 205, Cleveland, Ohio 44122 ("Care Bloom" or "Prize Provider"). Sponsor administers the Giveaway, entries, and LTCareNav accounts. Care Bloom is solely responsible for providing, shipping, activating, and supporting the Prize described in Section 6.

2. Giveaway Period

The Giveaway begins at 12:00 AM ET on October 1, 2026 and ends at the earlier of (a) the time the two hundred fiftieth (250th) eligible entry is received, or (b) 11:59 PM ET on December 31, 2026 ("Giveaway Period"). This Giveaway is a separate promotion. Entries in any other Sponsor giveaway do not carry over to this Giveaway, and entries in this Giveaway do not carry over to any other.

Two (2) winners will be selected in a single random drawing within seven (7) days after the Giveaway Period ends.

3. Eligibility

To be eligible to enter and win, an entrant must, at the time of entry and at the time of winner selection:

• Be at least 18 years of age or the age of majority in their state of residence, whichever is older;
• Be a legal resident of the fifty (50) United States or the District of Columbia;
• Hold an active LTCareNav family account in good standing (a free account may be created as part of entry — see Section 4), or have entered by the Alternate Method of Entry in Section 4;
• Have reliable home internet access at the residence where the CareBloom Kit would be installed, since CareBloom's gateway requires an active home internet connection to function;
• Have a care recipient who lives at a residence in the fifty (50) United States or the District of Columbia where the CareBloom Kit will be installed, and who, or whose legally authorized representative, is willing to complete Care Bloom's consent process at setup, including consent to being monitored and to the sharing of CareBloom data with the entrant's Care Circle;
• Have at least one designated person, other than the care recipient, who is willing and able to monitor the CareBloom app (this may be the entrant or another member of the entrant's Care Circle);
• Not be an employee, officer, director, or contractor of LTCareNav, Care Bloom, or their affiliates, or a member of such person's immediate family or household; and
• Not have previously won a CareBloom giveaway sponsored by Sponsor. Each household may win a CareBloom giveaway only once.

Sponsor reserves the right to verify eligibility at any stage and to disqualify any entrant who does not meet these requirements or who provides false or inaccurate information. Each potential winner must certify the requirements above in the Winner Affidavit described in Section 5.

4. How to Enter

No purchase necessary to enter. Online entry requires becoming an LTCareNav family member (free). You may also enter without creating an account by the Alternate Method of Entry below.

1. Complete the eligibility quiz at ltcarenav.com/winfreecarebloom, which includes the eligibility statements in Section 3 (state of residence of the entrant and the care recipient, home internet access, and having a designated person to monitor the CareBloom app);
2. Provide an email address at the end of the quiz;
3. Affirmatively agree to these Official Terms, the LTCareNav Terms of Use, the LTCareNav Privacy Policy, and the LTCareNav Consumer Health Data Privacy Disclosure.

Upon completing all three steps during the Giveaway Period, the entrant is entered into the Giveaway.

Alternate Method of Entry (AMOE). To enter without completing the online quiz or creating an account, hand-print your name, mailing address, email address, phone number, date of birth, and the state where the care recipient lives on a 3" x 5" card and mail it in a sealed, stamped envelope to: LTCareNav and Care Bloom 2026 Giveaway, PO Box 140, 30799 Pinetree Road, Cleveland, Ohio 44124. Mail-in entries must be received before the Giveaway Period ends and have the same chance of winning as online entries. Limit one mail-in entry per person and household; mechanically reproduced entries are void. A winner who entered by mail will be asked to complete the steps needed to activate the Prize.

Limit: one entry per person, per household. Multiple or duplicate entries are void.

5. Winner Selection and Notification

Within seven (7) days after the Giveaway Period ends, Sponsor will randomly select two (2) winners from all eligible entries received during the Giveaway Period, whether or not two hundred fifty (250) eligible entries have been received.

Potential winners will be notified by email and/or phone using the contact information provided at entry, within three (3) business days of the drawing. Each potential winner must respond and confirm eligibility within seventy-two (72) hours of notification, or an alternate winner may be selected. Each potential winner must also return, within seven (7) days after receiving it, a signed Winner Affidavit of Eligibility, Release, and Consent, confirming that the winner has the care recipient's authorization (or is the care recipient's legally authorized representative) as required by the Care Bloom Terms and Conditions, together with a completed IRS Form W-9. Failure to return these documents, or return of a notification as undeliverable, may result in forfeiture and selection of an alternate winner.

Odds of winning depend on the number of eligible entries received during the Giveaway Period.

6. Prize

Each of the two (2) winners will receive one (1) CareBloom Kit (smartwatch, five (5) wall-mounted sensors, five (5) anchors, and a home gateway) and the CareBloom subscription service at no cost for twelve (12) months from activation (the "Free Service Period"), for as long as the winner continues to actively use the product during that period (each, a "Prize"). The approximate retail value ("ARV") of each Prize is $2,388, based on twelve (12) months of subscription service at the standard $199 monthly rate. The total ARV of all prizes in this Giveaway is $4,776.

The CareBloom Kit carries no separate value because it is included with a paid subscription outside this Giveaway. Actual value depends on how long the winner uses the service.

The free subscription is subject to the winner's continued compliance with the Care Bloom Terms and Conditions. "Active use" means the CareBloom Kit transmits data to the CareBloom service at least once in any sixty (60) day period. If it does not, Care Bloom may end the free subscription after fourteen (14) days' email notice. If Care Bloom discontinues or materially changes the CareBloom subscription service for its customers generally, Care Bloom may end the free subscription on at least sixty (60) days' notice. Care Bloom's standard subscription payment terms, initial six-month commitment, cancellation fee, and refund and device-return requirements do not apply to the Prize, and the winner may stop using the service at any time without charge. For the Prize, Care Bloom may end the free subscription only as described in this Section 6 or for a material breach of the Care Bloom Terms and Conditions. The free subscription is personal to the winner's household and the care recipient named in the Winner Affidavit and ends if the CareBloom Kit is sold or transferred. After the Free Service Period, the winner may continue the service at Care Bloom's then-current rates or stop using it, with no obligation to return the Kit.

CareBloom is a remote caregiver monitoring service and is non-diagnostic. It is not a medical device, is not intended to diagnose, treat, or monitor any medical condition, and does not replace clinical care, in-person caregiving, or emergency services. CareBloom is not an emergency response or medical alert service and does not contact 911 or emergency responders. In an emergency, call 911. Prize fulfillment is contingent on the winner meeting the eligibility requirements in Section 3, including home internet access, having a designated person to monitor the app, and the care recipient's (or legally authorized representative's) consent.

The Prize is non-transferable and no cash or other substitution will be made. The winner is solely responsible for any applicable federal, state, or local taxes associated with the Prize. Sponsor or Care Bloom will issue an IRS Form 1099-MISC to the winner for each calendar year in which the value of the Prize provided equals or exceeds the IRS reporting threshold (currently $2,000).

Title to the CareBloom Kit passes to the winner on delivery. Care Bloom will ship the Kit to the eligible installation address within approximately thirty (30) days after the Winner Affidavit is verified and will provide its standard remote setup support. The Kit is provided "AS IS" and without warranty, as described in the Care Bloom Terms and Conditions. Replacement of lost, stolen, or damaged components is at Care Bloom's discretion and may be at the winner's cost. No return of the Kit is required when the free subscription ends.

7. Device, Service, and Liability Limitations

The CareBloom Kit and subscription are provided to the winner free of charge for the Free Service Period as described in Section 6. Use of the device and service remains subject to the Care Bloom Terms and Conditions (carebloom.com/terms-conditions), the Care Bloom Privacy Policy, and any other applicable Care Bloom agreements in addition to these Official Terms; in the event of a conflict specific to this Giveaway, these Official Terms govern.

CareBloom relies on the winner's home internet connection, the winner's own device settings, and the ongoing attentiveness of the designated monitor identified in Section 3 to function as intended. Neither Sponsor nor Care Bloom is responsible for missed alerts, delays, or service interruptions caused by internet outages, device malfunction, user error, or a monitor's failure to respond. CareBloom is intended to supplement, not replace, in-person caregiving, supervision, and emergency response. The winner, the designated monitor(s), and the care recipient remain solely responsible for the care recipient's supervision, care, and safety.

8. Publicity Release

As a condition of accepting the Prize, each winner must sign a publicity release, except where prohibited by law, consenting to Sponsor's use of the winner's first name, city/state, and statements regarding their experience for promotional purposes related to this Giveaway, without additional compensation, unless otherwise required by law. Winners may also be asked, but are never required as a condition of receiving or keeping the Prize, to write a review of their CareBloom experience. Any winner who posts or provides a review or endorsement must clearly disclose that they received the CareBloom Kit and subscription free through this Giveaway. Sponsor will not use the care recipient's name, image, or health information without the separate written consent of the care recipient or their legally authorized representative.

9. Privacy

Information collected in connection with this Giveaway will be handled in accordance with the LTCareNav Privacy Policy and Consumer Health Data Privacy Disclosure. By entering, entrants consent to the collection and use of their information as described there and in these Official Terms. Sponsor will share each winner's and care recipient's name, contact information, and installation address with Care Bloom solely to fulfill and support the Prize; after activation, CareBloom device data is handled under Care Bloom's applicable terms and privacy policy. An entrant who deletes their LTCareNav account, or withdraws consent to the processing of their consumer health data, before the drawing is no longer eligible to win.

10. Release and Limitation of Liability

By entering, entrants agree to release and hold harmless Sponsor, Care Bloom, and their respective parents, subsidiaries, and affiliates, and each of their respective officers, directors, members, managers, employees, and agents (the "Released Parties"), from any and all liability, injury, loss, or damage arising from participation in this Giveaway or acceptance, receipt, use, or installation of the Prize, including any liability related to the operation or non-operation of the CareBloom device or service. See also Section 7. Use of the CareBloom Kit and service by the winner, the care recipient, and Care Circle members is also subject to the disclaimer, limitation of liability, and indemnification provisions of the Care Bloom Terms and Conditions. The Disclaimer and Limitations on Our Liability and Indemnification provisions of the LTCareNav Terms of Use (Sections 17 and 18) also apply to this Giveaway. TO THE FULLEST EXTENT PERMITTED BY LAW, THE RELEASED PARTIES WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM THE GIVEAWAY OR THE PRIZE. Nothing in these Official Terms limits liability that cannot be limited under applicable law, including for gross negligence or willful misconduct.

11. Right to Cancel, Modify, or Disqualify

Sponsor reserves the right, at its sole discretion and at any time, to cancel, suspend, pause, modify, amend, or end the Giveaway, or these Official Terms, for any reason, including if the Giveaway is not capable of running as planned due to fraud, technical failure, or any other cause. If Sponsor elects to end the Giveaway early, Sponsor will provide reasonable notice on the Giveaway page. Sponsor reserves the right to disqualify any entrant found to be tampering with the entry process or acting in violation of these Official Terms. Sponsor will not modify these Official Terms in a way that materially reduces an entrant's chance of winning while the Giveaway is under way, except as required by law or to address fraud or technical failure, and will post any change with a revised "Last updated" date. Ending the Giveaway does not affect a Prize already awarded, which continues as described in Section 6.

12. Governing Law and Disputes

This Giveaway and these Official Terms are governed by the laws of the State of Ohio, without regard to conflict-of-law principles. Any dispute arising out of or relating to this Giveaway or the Prize will be resolved under the Dispute Resolution & Arbitration provisions (Section 19) of the LTCareNav Terms of Use, including binding individual arbitration administered by the American Arbitration Association, the class action and jury trial waivers, the informal dispute resolution requirement, and the opt-out procedure described there. Any dispute relating to the operation of the CareBloom Kit or service after activation is governed by the Dispute Resolution & Arbitration provisions of the Care Bloom Terms and Conditions. To the extent any claim may proceed in court, exclusive venue is in the state or federal courts located in Cuyahoga County, Ohio. Care Bloom is an intended third-party beneficiary of this Section.

13. Void Where Prohibited

This Giveaway is void where prohibited or restricted by law.

15. Winners List

For the name(s) of the winner(s), send a self-addressed, stamped envelope marked "LTCareNav and Care Bloom 2026 Giveaway Winners List" to the address in Section 14 within sixty (60) days after the Giveaway Period ends.

16. Relationship to LTCareNav Terms of Use

These Official Terms supplement the LTCareNav Terms of Use. If they conflict on a matter specific to this Giveaway, these Official Terms control.
